Researchers at Boston Children’s Hospital have found that modern vaping devices are delivering increasingly heavy doses of addictive nicotine to teenagers. Published in JAMA Psychiatry, the study reveals that average urine levels of cotinine—the primary metabolite of nicotine—nearly tripled among adolescent patients over a span of just a few months in late 2024.

The research team, led by Dr. Sharon Levy, chief of addiction medicine at Boston Children’s Hospital, analyzed the health records of 150 patients aged 13 to 19 who were treated for substance use disorders between 2023 and 2025. The dataset included more than 1,900 urine test results.

The data showed a sharp, abrupt spike in nicotine exposure. Prior to October 2024, the average maximum cotinine concentration in the teens’ urine was 1,062 nanograms per milliliter (ng/mL).
